Community Action Partnership for Madison County is a private, non-profit, anti-poverty agency that assists individuals and families through a variety of programs. Established in 1983, the agency provides services in Madison, Herkimer, and Oneida counties from seven office locations.  Some of the primary programs offered by the agency are  Early Head Start, Healthy Families,  Mentoring, Supportive Services, Transitional Housing,  and WIC.  CAP is an EOE/AAE.


 

Healthy Families NY Family Support Worker 

We are hiring a Family Support Worker for our Healthy Families Program. This position would be based out of our Oneida office.

Job Summary:

  • Provide home visiting services to families following the guidelines established by Healthy Families New York.
  • Initiate and maintain regular contact with families; provide prenatal and other health information; assist to strengthen parent/child relations; introduce new resources; make appropriate referrals that assist the family in achieving their goals and outcome measures to demonstrate success.
  • Maintain accurate and timely documentation as required by the program and maintain the confidentiality of all case records and information gathered in the course of providing services.
  • Participate in weekly individual supervision. Participate in staff training, case consultation, staff meetings, and case coordination meetings with other community service providers.
  • Prepare for home visits by organizing approved program curriculum, individualized resources, and supplies, and required paperwork.

 Qualifications and Skills:

  •  Minimum qualification – High school diploma / GED plus three years’ work experience caring for young children or providing family development services required; Preferred – Child Development Accreditation or Associates degree in human services or related field and at least 1-year’s experience working with families and/or children.
  • Experience working with or providing services to children and families or working with, or assisting, high-risk families in a community setting is preferred.  Credentialed Family Development Worker preferred.
  • Must have a valid driver’s license evidencing safe driving history and a reliable vehicle.

Housing Counselor -STEHP/Transitional Housing

 JOB SUMMARY:

• Describe Agency Housing programs to potential participants, landlords, and community.  Conduct community outreach for the program as requested.

• Assist clients with affordable housing search, facilitate move-in, including but not limited to gathering furnishings, reviewing the lease, reviewing tenant responsibilities, meeting with landlords, and discussing payment requirements. 

• Provide intensive case management with clients based on the need of the individual /family.

• Assist the Housing Program Manager in maintaining accurate reporting of outcomes and with consumers including demographics of population served.  Complete paperwork and case recording in compliance with federal, state and local requirements.  

• Assist in the inventory, organization and setting up of Transitional Housing apartments. 

QUALIFICATION AND SKILLS:

• High school degree plus five years’ experience in human services or an Associate’s degree or Bachelor’s degree with 2-3 years’ experience.   

• Must have a working knowledge of computers and software such as Microsoft Word, Excel, and data entry.  Experience with a Homeless Management Information System (HMIS) preferred.

• Must possess strong oral and written communication skills as well as organizational and time management skills.

• Must be able to identify and nurture strengths and have the ability to establish trusting relationships with culturally diverse populations. 

• Must be able to lift items up to 15 pounds up and down stairs. 

•Must have a valid driver’s license evidencing safe driving history and a reliable vehicle.

WIC Nutrition Assistant 

We are hiring a WIC Nutrition Assistant.  This is a full-time position, eligible for benefits.  This position will be based in the Herkimer WIC office, with travel to other Tri-County WIC offices as needed.

Duties:

Provide support to the Tri-County WIC initiative.  Provide clinic and office support to the WIC Nutritionists.  Gather information for certifications and nutrition assessments in a consumer-friendly, professional, and participant-centered manner with the ultimate goal to enhance the lives of those served. 

  • Provide clinic and office support
  • Collect information for certifications, including verifying income and adjunct qualifiers
  • Collect information for the nutrition assessment, including obtaining anthropometric and hematological data, as trained
  • Participate in breastfeeding promotion and support
  • Provide referrals to health and social services as required
  • Assist Nutritionist in planning and evaluating participant-centered nutrition education

Requirements:

  • High School Diploma or GED and one year of experience working with children and families
  • Associates or Bachelor’s Degree in Health or Human Services can be substituted for experience
  • Must possess strong oral and written communication skills
  • Must possess the ability to work independently and as a team member, the ability to keep information confidential, and the ability to establish healthy client/worker boundaries
  • Computer proficient, especially in Microsoft applications
  • Must have a valid driver’s license, safe driving history, and reliable vehicle
  • Required to complete Family Development Credentialing course within three years of hire, as available

WIC Qualified Nutritionist (QN)

We are hiring a WIC Qualified Nutritionist (QN).  This is a full-time position, eligible for benefits.  This position will be based in the Oneida WIC office, with travel to other Tri-County WIC offices as needed. 

Duties:

Guide WIC participants throughout the nutrition and breastfeeding assessment and education process throughout the Tri-County clinics. Provide consumer friendly, professional, participant-centered quality care that will enhance the lives of those served.  Provide nutrition counseling and education to participants in both individual and group settings following the NYS Program standards and policies.   Provide participant-centered nutrition counseling and education to high-risk participants. Have the ability to perform all clinic functions. 

 

  • Set the tone for excellent customer service to participants in the clinic.
  • Conduct a complete nutrition assessment, determine nutrition risk and enroll participants.
  • Prescribe, tailor, authorize, issue and reissue all food packages.
  • Promote Breastfeeding as the preferred infant feeding method following the Loving Support Model guidelines. 
  • Conduct a breastfeeding assessment, and provide participant-centered counseling that helps a mother initiate breastfeeding, establish a milk supply, exclusively breastfeed and continue breastfeeding. Address common breastfeeding concerns, issue breast pumps and refer to the breastfeeding coordinator when necessary.
  • Provide and document participant-centered nutrition counseling and education in both individual and group settings for participants.
  • Provide referrals to health and public assistance programs and document follow-up.
  • Review and approve formula requests requiring medical documentation in consultation with the Health Care Provider.
  • Develop and implement high-risk care plans and oversee high-risk care to all participants. Update and close high-risk care plans. Refer high-risk participants to other health-related and social services, as necessary. Track high-risk participant’s progress in improving their health and document for outcomes.
  • Ensure the efficient flow of the nutrition assessment and education process.
  • Develop, conduct and evaluate nutrition training for Local Agency staff and participants in nutrition education topics under the leadership of the Nutrition Team Leader. 
  • Provide guidance to the Nutrition Assistant staff in collecting nutrition assessment data.
  • Provided guidance in nutrition assessment and counseling/education to CPAs as needed.
  • Rotate to other clinic sites as required.
  • Coordinate and participate in Outreach and Public Health Detailing.

Requirements:

  • Minimum Bachelors or Masters in Nutrition.
  • Registered Dietitian Nutritionist (RDN) is preferred. 
  • Must possess strong oral and written communication skills, the ability to work independently and as a team member, ability to keep information confidential, and the ability to establish healthy client/worker boundaries.
  • Proficient in use of computer and Microsoft applications (particularly Word); experience with automated databases preferred. 
  • Valid driver’s license and reliable transportation.
